Job Title: Case Manager Job Location: New Jersey, United States of America Job Location Type: Remote Job Contract Type: Full-time Job Seniority Level: Job title: Case Manager • Location: Remote, US (Territory: Florida-Puerto Rico) About the Job: As a Case Manager, you will deliver exceptional service to patients and families throughout their therapy journey through empathetic listening, compassionate care, and unwavering commitment. You will serve as the vital connection between patients, families, healthcare providers, insurance companies, specialty pharmacies, and local resources. Your responsibilities will include conducting benefit investigations, providing patient education, enrolling patients in support programs, and coordinating drug delivery. You will excel at creating positive patient experiences and become a trusted resource within the rare blood community. As an active cross-functional team member, you will help deliver best-in-class services that ensure seamless access to therapy. In Specialty Care, you ll help deliver breakthrough treatments that bring hope to patients with some of the highest unmet needs. Main Responsibilities: • Engage patients and families to build long-term relationships, uncover needs, understand challenges, and provide education and support to initiate or continue therapy; manage complex issues and utilize approved resources to reduce or remove barriers as needed • Assist in obtaining insurance approvals for therapy and proactively plan for ongoing coverage maintenance by collaborating with patients, families, healthcare providers, insurance companies, and specialty pharmacies. • Enroll eligible patients in financial assistance or gap coverage programs • Educate patients, families, and healthcare providers on available patient support services, insurance options, limitations, and requirements; connect patients and families with relevant local resources • Ensure close cross-functional collaboration with Patient Support Services field teams, Sales, and Market Access partners as appropriate to deliver a consistent, integrated approach to patient access • Establish and maintain professional, effective relationships with all internal and external stakeholders, including Patient Support Services colleagues, Medical, Sales, Market Access, Patient Advocacy, insurance case managers, specialty pharmacies, home health agencies, infusion site staff, and office coordinators • Represent Sanofi at patient congresses and meetings as a trusted community resource • Document call details with a strong focus on data integrity to ensure compliance and continuously enhance our service offering • Leverage CRM reporting tools and data analytics to make strategic, territory-level decisions and prioritize patient and customer needs • Provide caseload coverage outside of assigned territory as needed • Actively seek coaching and feedback to continuously enhance service skills • Ensure full compliance with Sanofi policies and guidelines • Professional-level Spanish proficiency (verbal and written) required to communicate directly with patients, families, and healthcare providers in Florida and Puerto Rico About You: You are a passionate, detail-oriented professional with a strong background in patient-facing roles. You thrive in fast-paced environments, excel at building meaningful relationships, and are deeply committed to improving the lives of patients living with rare and chronic conditions. Preferred Qualifications: • Bachelor s degree required • Minimum 2+ years patient-facing role experience in case management, reimbursement, patient advocacy, or provider s office • Experience with engaging patients and caregivers managing rare or chronic conditions • Professional-level Spanish proficiency (verbal and written) • Comprehensive understanding of the reimbursement process, billing and coding, insurance plans, payer trends, financial assistance programs, charitable access and related resources • Excellent phone communication with a strong service-oriented mindset • Exceptional attention to detail with the ability to multi-task effectively in a fast-paced environment • Experience with Salesforce CRM preferred • Proficient in Microsoft Word, MS Excel, and PowerPoint • Ability to travel within the US, including occasional nights and weekends (up to 10%) • Flexibility to adjust working hours based on business needs and team coverage requirements.
